FIRST TIME REGISTRANTS:
- Please provide a photocopy of the player's birth certificate.
- Please provide photocopies of two proofs of New Westminster residency.
- Fill in either the NWMHA Player registration form or the Juvenile registration form
- If you have never registered for hockey in the lower mainland, please download the attached PCAHA Registration form and enclose with your registration: PCAHA Registration
- If you have been previously registered in another hockey association but are new to New Westminster, please complete the following PCAHA Application for Player Movement form: PCAHA Application for Player Movement
- Residential Declaration Form - This form is for any player who has made a residential move and is now registering in a different association than the previous year.
- If your child was NOT born in Canada, further proof of Citizenship is required from the parent(s) in the form of a passport, Visa Card or Residence Card.

Family Rate
For three or more players the first two are full fee and the 3rd is $100.00.
Goalie Fee: If he/she has all their own equipment, the fee is 1/2 price.
Completed registration forms can be dropped off at Queen's Park Arena or Moody Park Arena during regular office hours, or can be mailed to the Association post box.
